Stable monotherapy with clozapine or olanzapine increases the incidence of diabetes mellitus in people with schizophrenia.

چکیده
Risk factors: Participants diagnosed with diabetes mellitus or hospitalised with diabetic ketoacidosis were identified from the Department of Veterans Affairs and were predominantly older and male. Time to diagnosis and hospitalisation was modelled using the Cox proportional hazards models. Attributable risks of each outcome associated with different antipsychotics were calculated. The five antipsychotics groups analysed were: clozapine, olanzapine, quetipane, risperidone, and control (all conventional antipsychotics).

Incidence of newly diagnosed diabetes attributable to atypical antipsychotic medications.
OBJECTIVE The purpose of the study was to determine the proportion of patients with schizophrenia with a stable regimen of antipsychotic monotherapy who developed diabetes or were hospitalized for ketoacidosis. METHOD Patients with schizophrenia for whom a stable regimen of antipsychotic monotherapy was consistently prescribed during any 3-month period between June 1999 and September 2000 and...
متن کاملAntipsychotic exposure and type 2 diabetes among patients with schizophrenia: a matched case-control study of California Medicaid claims.
PURPOSE To examine the risk of developing type 2 diabetes mellitus among people with schizophrenia exposed to atypical antipsychotics (clozapine, olanzapine, quetiapine, risperidone) compared to those exposed to conventional antipsychotics. METHODS A matched case-control design was used to examine California Medicaid beneficiaries. Cases developed diabetes subsequent to being diagnosed with s...
متن کاملClozapine use in patients with schizophrenia and the risk of diabetes, hyperlipidemia, and hypertension: a claims-based approach.
BACKGROUND Numerous case reports have linked clozapine to the development of diabetes mellitus and hyperlipidemia in patients with schizophrenia. However, investigators have been unable to clearly demonstrate this association when compared with a control group receiving conventional antipsychotics. METHODS Medical and pharmacy claims from the Iowa Medicaid program were used to compare inciden...
متن کاملGlucose metabolism in patients with schizophrenia treated with atypical antipsychotic agents: a frequently sampled intravenous glucose tolerance test and minimal model analysis.
BACKGROUND While the incidence of new-onset diabetes mellitus may be increasing in patients with schizophrenia treated with certain atypical antipsychotic agents, it remains unclear whether atypical agents are directly affecting glucose metabolism or simply increasing known risk factors for diabetes. OBJECTIVE To study the 2 drugs most clearly implicated (clozapine and olanzapine) and risperi...
متن کاملWhat evidence is there to show which antipsychotics are more diabetogenic than others?
BACKGROUND The use of antipsychotic therapy has been proven to have an association with the incidence of diabetes mellitus. The use of atypical antipsychotics is shown to have a higher association, in contrast with typical antipsychotics. Olanzapine and Clozapine appear to have the highest rates of diabetes mellitus incidence, due to their tendency to affect glucose metabolism compared with oth...
